Below is the preparticipation physical examination schedule for this fall. Physicals will only be given to new, incoming athletes to UIndy (i.e. freshman, transfers, walk-ons, etc) on these dates. The complete physical exam will cost $55.00, payable at the time of the physical. There will be an echocardiogram screen performed by The Care Group - St. Vincent Hospital which is included in the cost of the physical exam. The time indicated after your sport is your reporting time, please report to the west side of Nicoson Hall at the check in table to begin the exam. It should take each athlete approximately 60 minutes to complete the physicals system. Each new athlete should bring with them their completed health history form folder you received in the summer, Assumption of Risk form, Consent of Medical Information Release, Consent to Treat and Parent Insurance Information forms and be dressed in shorts and a T-shirt for the exam.
All new/transfer athletes to UIndy must complete a preparticipation physical exam with UIndy team physicians prior to beginning any activity.
Please report to the West side of Nicoson hall on your assigned date and time listed below. Cash or checks to UIndy Athletics for $55 will be required prior to beginning the exam.
