UIndy Athletics

Welcome to the University of Indianapolis

Dear Greyhound Team Member,

I would like to begin by welcoming you in advance to the University of Indianapolis and wishing you luck in your upcoming season.  All of us in the Athletic Training Office hope it will be a safe, fun and successful athletic year.

Preparticipation physical examinations are required prior to any athletic activity.  Further more it is an NCAA requirement that UIndy team physicians complete this examination.  Please refer to the Athletic Training link from the Athletics web page to review the preparticipation physicals schedule.  Along with the athletics physical, each athlete will receive an echocardiogram screening.  More information about this screening may be found on the next page. The preparticipation physical examination and echo screening will be given for a fee of $60.  Payment may be received in cash, credit card or personal check made out to The University of Indianapolis.

Information on the UIndy's athletic insurance policy can be found on the next page.  Please go to the Athletic Training link from the UIndy Athletics web page to down load and complete the form and return them to the Athletic Training Office with your health history questionnaire. 

The University of Indianapolis Athletic Training Medical History Questionnaire form is to be completed in full by your parents with all information being as accurate and current as possible.  This will provide our medical staff with the appropriate information to complete your physical exam.  This form must be sent to you by your head coach.

The Admissions Office will also be sending you a Student Health Record form which needs to be completed.  You may leave the Physical Exam (part IV) blank (please list your sport).  Your athletic physical will fulfill this part.  The nurse practitioners will review and send you a letter if it is not completed or up-to-date.  It is Indiana law to have immunizations current.  If your immunization record is not updated by the time of your athletic physical, you are expected to get this done ASAP in the Student Health Center for the fees listed here.  Payment is expected that day (cash, charge or check to University of Indianapolis).  If not paid within 10 days, you will be dropped from classes.  The fees are as follows:  Tb skin test $5, MMR $60, Tetanus $45, Hepatitis B $45 each, Meningitis $100, HVP Vaccine $125.  For questions, see the Student Health Services web sit, or leave a message on the Student Health Center’s voice mail at 317-788-3437. Someone will return your call within a week.

Please return the Health History Questionnaire and Parent Insurance Information Form fully completed, by July 31. 

If your insurance should change during the season, please notify the athletic training office as soon as possible.  Every student athlete must have insurance in order to participate!!  A photo copy of the front and back of your insurance card attached to the Parent Insurance Information form enclosed will move the process along faster.

Your immunization card and or immunization fee and physical examination fee may be turned in at the time of your physical examination.

UNIVERSITY OF INDIANAPOLIS STUDENT-ATHLETE MEDICAL INSURANCE

Accidents and injuries do occur, and we attempt to provide UIndy athletes with the very best possible care.  Medical bills are incurred when the athlete is treated, whether it is locally or during away competitions.  The NCAA does not permit the University of Indianapolis or any college/ university to provide coverage or to pay bills incurred for expenses related to illnesses or conditions which are not sustained as the direct result of an accident or injury in an intercollegiate sports program.  Each athlete is required by the UIndy Athletic Department to provide their own primary insurance.  No athlete will be permitted to participate in UIndy athletics with out proof of personal medical insurance.

Please complete the enclosed Parent Insurance Information form in it’s entirety.  Please note, it is important you provide correct and detailed insurance information to facilitate your medical care.  INCLUDE A COPY OF YOUR INSURANCE CARD.  With the predominance of HMO’s and PPO’s, it is essential we know your policies and how they work.  The UIndy cannot be responsible for bills based on false or non-current insurance information.

The student-athlete insurance program at the University of Indianapolis is purchased through AG Administrators Insurance.  It provides secondary coverage for athletic accidents or injuries while participating in the play of official practice and/or competition of intercollegiate sports during the 23 regularly-scheduled sports seasons/activities.  The AG Administrators insurance policy covers only those injuries sustained during participation in these athletic events/ activities.  All official practices or competitions due to post-season tournaments are covered.  Injuries sustained in voluntary conditioning or play, or during unofficial practices, are not covered by the AG Administrators athletic insurance policy.  Injuries/conditions that are deemed preexisting, chronic, disease in nature or aggravated by athletic activity are not covered.  It is, as stated above, secondary insurance.  It is, therefore, mandatory that you complete and return the Parent Insurance Information Form.  The insurance coverage provided by you is the PRIMARY insurance.  Payment of bills past 52 weeks from the date of injury will be denied per limitations set by the AG Administrators insurance policy


PREEXISTING INJURIES OR CONDITIONS

The University of Indianapolis will not be responsible for preexisting injuries or conditions that occurred prior to participating in UIndy athletics.  Disqualification due to a preexisting problem is at the discretion of the University of Indianapolis team physician(s).  Failure to report and document preexisting problems releases University of Indianapolis from any liability in the event of another problem caused by the initial problem.  The University of Indianapolis will not be financially responsible for medications for preexisting conditions, or for non-athletic injuries or conditions such as asthma, allergies, diabetes, etc.  These conditions should be reported to the Student Health Service and will be dealt with as non-athletic injuries or conditions.
